You can add a title to an image, but that title is only used within iPhoto. To edit a photo's title: Select the photo. If you don't see the photo's information, click the Information button (shown above) in the bottom-left corner of the iPhoto window. Type a new title in the Title field. this info is directly from the iPhoto Help menu. While in iPhoto, click Help on the menu bar and type in any question you have or look at the table of contents.
